Completing forms before your visit gives the clinical team time to review your health history, medications, allergies and previous dental care. Select the English or Spanish version you prefer, print the completed documents and bring them with you. Do not email completed forms unless our staff provides a secure method.

Before your first appointment
Bring a photo ID, your dental insurance card when applicable and a current list of medications and supplements. A parent or legal guardian should accompany a minor and complete the required consent documents. Tell the team about medical conditions, pregnancy, medication changes or accommodations that may affect your visit.
